CONFIDENTIALITY

Confidential means that the research records will include some information about you, and this information will be stored in such a manner that some linkage between your identity and the response in the research exists. We will use Qualtrics, an online survey platform, to collect and forward your responses to us. We will know your IP address when you respond to the online research. We will ask you to include demographic information when you complete the survey. Your IP address and identifiable information will not be stored with your responses. Instead, your responses will be assigned a subject #, which will be stored separately from your responses so others will not know which responses are yours. We will securely store the key code linking your responses to your identifiable information in a separate password protected file which will be destroyed after data analysis is complete and study findings are professionally presented or published. We will download your responses to a secure file that is stored using a secure platform (Box) accessible only to the study team. There is no plan to delete the responses. We plan to study the data for some time.
