2017 New Jersey Health Information Technology Survey

On behalf of the New Jersey Innovation Institute (NJII) and the New Jersey Health Information Network (NJHIN), we are asking for your help on an important study surveying healthcare providers and their office managers (or equivalent) about their utilization of electronic health records (EHR) and other health information technology (HIT) in clinical practice. Even if you do not use an EHR or health information technology, we would still like to hear from you!

This important research is being conducted to provide the State of New Jersey an understanding of healthcare technology utilization, which will assist them in developing the State Medicaid Health Information Technology Plan (DMAHS).

This survey is being sent to all physicians, dentists, nurse practitioners, and optometrists in the state of New Jersey. The survey takes approximately 15 minutes to complete.

This research is confidential. Confidential means that the research records will include some information about you, and this information will be stored in such a manner that some linkage between your identity and the response in the research exists. Some of the information collected about you includes information about your age, education and sexual identification.

Please note that we will keep this information confidential by limiting access to the research. All data we collect and share with representatives at NJII will not be linked to your name and contact information. Additionally, your name and contact information will be stored separately from your responses in an approved password-protected file.

There are no foreseeable risks to participation in this study.

Your participation is voluntary. You may end the survey at any time and you may skip questions you do not want to answer.
